Your primary care consultant will serve as your attending doctor for any hospitalization. If your doctor is not available during your hospital stay, his/her counterpart will be designated as your attending consultant and will oversee your care..
As part of the admissions process, you will be given the opportunity to appoint someone with whom your doctor can discuss your care. In order to have someone make medical decisions on your behalf if you are unable to communicate, you will need to complete a Healthcare Power of Attorney form. It is advisable to be familiar with this and other legal documents such as a Living Will.

Lifenest Hospital asks that any cancellations be made at least 72 hours prior to the scheduled appointment. This will allow for time reallocations to available patients. Cancellations may be made by sending an email or by calling us on 08035341469
Lifenest Hospital asks that new patients arrive 15 - 30 minutes prior to their scheduled appointment to allow adequate time for the registration process. For subsequent visits, patients are asked to arrive 5 - 10 minutes in advance of their scheduled appointment.

Patients should bring the following information: Health insurance ID or personal ATM debit card Referral forms from your physician (if required by your insurance carrier) Government issued identification Driver's license Pre-admission form (if not previously submitted)
Please call the Hospital and request to speak with Nurses Station for any questions or concerns that you might have regarding your hospital stay.
Patients are advised to bring photo identification, a copy of your insurance card, toiletries, sleepwear, and personal items. It is also recommended that you bring a complete list of all medications including dosage and frequency with you.
Throughout your hospital stay you are encouraged to discuss your care with your attending physician. You may also request to speak with the Hospital Administrator or Patient Experience officer about any concerns that you may have with your treatment plan or hospital stay.
